2025 Peter Hort Quality of Life Cancer Grants Guidelines
PURPOSE:
The Peter Hort Quality of Life Cancer Grants (PHQOL) are dedicated to supporting cancer patients by ensuring they are not alone during treatment. Our goal is to strengthen patients’ emotional support networks and financial resources, enabling them to face cancer with the added resilience that comes from the presence of their loved ones. Rema Hort Mann (RHM) typically funds travel expenses, phone/internet services, childcare, and hotel accommodations—all with the intent of bringing loved ones to the patient’s side during treatment.
ELIGIBILITY:
- Grants are specifically intended to connect patients with loved ones for emotional support during treatment.
- Patients must reside in the greater New York Metropolitan area.
- Applicants must be currently undergoing cancer treatment.
- Priority is given to patients with limited emotional support networks and financial resources.
- Due to limited resources, grants are generally reserved for the travel of a single person.
- Grants do not cover medical expenses or living expenses such as rent or food.
- Grants are not awarded retrospectively.
GUIDELINES:
- Grant applications must be submitted by pre-registered social workers on behalf of patients.
- Applications are accepted on a rolling basis.
- A Social Security number is required to process payment.
- Only one application per patient may be submitted.
APPLICATION REQUIREMENTS:
- Social worker's name and email address.
- Patient’s name, address, and the following details:
- Social Security number
- Borough of residence
- Gender identity (optional)
- Racial/ethnic group (optional)
- Age (optional)
- Amount of funding requested with an itemized list of expenses.
- Check/funding mailing address; a safe and reliable place where funding can be delivered.
- A description (300 words max) of how this grant will support the patient during treatment.
- List the type of cancer and current treatment details.
- A one-sentence summary of the grant request.
EXAMPLES OF PREVIOUSLY AWARDED GRANTS:
- Childcare: Claudette, a 43-year-old single mother undergoing a stem cell transplant for leukemia, received a grant to cover childcare expenses, allowing her mother to be by her side during treatment.
- Travel: Serena, a 47-year-old single mother diagnosed with breast cancer, received a grant to cover travel costs for her closest friend to be with her during her treatment in New York City.
- Hotel: Julie, a 44-year-old woman recovering from ovarian cancer surgery, received a grant to cover hotel expenses so her mother from California could stay with her and support her during chemotherapy treatment.
